Consider Approval of Amended and Restated Consultant Service Contract for Youth Investment Program with Youthprise and Northfield Healthy Community Initiative related to the Northfield Mayor's Youth Council and the Northfield Alliance for Substance Abuse Prevention.

Action Requested:
The Northfield City Council approves the amended and restated contract with YouthPrise in partnership with Northfield Healthy Community Initiative for the Northfield Youth Investment Program.

Summary
The Northfield City Council approved a contract with YouthPrise in partnership with the Northfield Healthy Community Initiative (HCI) on July 17, 2018. Youthprise and HCI requested a few items in the contract be amended. The proposed amendments are shown on the attached amended contract and include the following:
* Corrected Healthy Community Initiative's name in two places.
* Section IV - Term and Termination - B. Termination. Change termination clause from sixty (60) days to thirty (30) days written notice on page 5.
* Updated contact person and address for Youthprise on pages 1& 8.
* Updated Scope of Services - Exhibit 1. The wrong scope of services was inadvertently used when staff brought the original contract forward for approval. HCI had proposed a revised scope. The revised scope aligns with the Youth First booklet that was included with the approval.

Staff has reviewed the amendments with the City Attorney and recommends approval of the amended and restated contract.

Financial Impacts:
The amendments do not have any additional financial implications. The contract provides for $51,000.00 for 2019, $52,020 for 2020, and $53,060 for 2021, with an additional $12,500/year for expenses related to community education for Northfield ASAP initiatives.

The contract term is from January 1, 2019 through December 31, 2021...
